U.S. Department of Education Year 2002 Elementary and
Secondary School Civil Rights Compliance Report
The Office for Civil Rights (OCR) of the U.S. Department of Education is
conducting its survey for the Year 2002 Elementary and Secondary School
Civil Rights Compliance Report.
The Education Information Services Office, using data from 2001-02
Survey 5 and 2002-03 Survey 2, will submit the majority of the data for
the report to OCR. However, since additional data are needed to satisfy
OCR’s reporting requirements, the following three forms must be
completed:
1.

Form ESE 546, Year 2002 Elementary and Secondary School Civil
Rights Compliance Report District Summary and its instruction sheet.
Each district must complete this form.
2.
?
Form ESE 761, Interscholastic Athletics School Report.
This form
must be completed by all high schools that have interscholastic
athletic programs.
3.
?
Form ESE 630, Ability Grouping.
Each school must complete this
form.
The data submitted on these reports, along with appropriate school data
from Survey 5 and Survey 2, will be used to create a data tape to be sent to
OCR that will include your district’s 2002 Civil Rights Compliance
Report.

has been created to aid you in

reporting these data. The site is divided into two sections: OCR data will be reported on
€
District Summary, Ability Grouping and Interscholastic Athletics School Report Forms. In
€
conjunction with the Office of Multicultural Student Language Education (OMSLE), a
€
second form must be completed regarding magnet schools or programs and student
€
participation, including Limited English Proficient students (LEP). A portion of this form
€
will be used for completing OCR requests. Your district ESOL contact person already has the
€
logon and password needed for access to the webite. This will be the same logon and
€
password used for the two previous immigrant surveys.

As stated earlier, the majority of OCR reporting requirements can be met using data from
Survey 5 and Survey 2 already reported on the DOE Information Data Base, which covers
basically all of the requirements for OCR’s school level form, the ED102 - Individual School
Report. Copies of this form are not being sent due to its length. However, if you would like
to have a copy of the ED102 mailed to you, please contact Nada Stauffer at (850) 487-2280
or SunCom 277-2280 or at Nada.Stauffer@fldoe.org.
All forms are to be submitted by March 7, 2003.
A report will be created which your district’s MIS staff may download. Notification will be
made of the availability of the report. If you or your staff have any questions, please contact
