RE: BEAR Form Submission

 

 

BEAR form submission: Do NOT assume that this form has been completed if

you are required to do this to receive your discounts; please check your records

or with whomever does this for you. A quick check of the SLD database indicates

that Florida has not submitted for all the "committed monies." I would like to ask

that all entities that received approved FRNs for Year 4 (2001-2002) to examine

whether or not they submitted a Form 472, if required. Example: Generally Alltel

provides the discounts on their bills. So, if your service provider has been

providing you the discounts already, you do not have to submit a BEAR form.

Those applicants working with the Florida Learning Alliance, on the items that

FLA has approved FRNs for your district, do not submit a Form 472 (BEAR).

 

It is important that you meet this deadline. If you miss it, it is highly unlikely that

you will be allowed to receive your monies. So, for this year and any previous

years, that might have missed a BEAR submission, let’s work on a response to

request an exception. A very good reason will be needed for the exception. The

best practice would be to submit correctly on time before the deadline, as

missing deadlines are hard to overcome.
