In accordance with the Office for Civil Rights (OCR) of the U.S. Department of Education and the Florida Department of Education’s strategic imperative to increase the quantity and improve the quality of education options for all students, a survey for the Year 2008 Elementary and Secondary School Civil Rights Compliance Report is being conducted.
    9. The Education Information Services Office, using data from 2007-08 Survey 5 and 2008-09 Survey 2, will submit the majority of the data for the report to OCR. However, since additional data are needed to satisfy OCR’s reporting requirements, a website (http://data.fldoe.org/ocr_report/) has been created to aid you in reporting these data. Please access this website and complete the District Summary, Ability Grouping, Advanced Placement, Alternative School and Interscholastic Athletics School Report Forms. Your user name is your district name and your password is your 2-digit district number. After your initial access of the website you will be asked to change your password to one that you create.
    10. The data submitted on the website, along with appropriate school data from Survey 5 and Survey 2, will be used to create a data file to be sent to OCR that will include your district’s 2008 Civil Rights Compliance Report.

As stated, the majority of OCR reporting requirements can be met using data already reported on the DOE Information Data Base, which covers basically all of the requirements for OCR’s school level form, ED102 - Individual School Report. Copies of this form are not being sent to you due to its length.
    16. A report will be created which your district’s Information Services staff may download. You will be notified of the availability of the report. If you or your staff have any questions or you would like a copy of ED102, please contact Nada Scott in Education Information Services, at (850) 245-0400 or at Nada.Scott@fldoe.org .
    17. The website survey forms must be completed by March 13, 2009.
