    APPLICATION CHECKLIST
    (For applicant use only; do not send with application.)
     
    General:
    All audio-visual footage is in raw form – no editing, adding graphics, etc.
    All word limits have been adhered to, and all text is single-spaced and in Times New Roman 12-point font (regular).
    Criterion I:
    The Identification Form is complete.
    A copy of the Principal’s Letter of Intent to Apply, with all required information, is included; and the hard copy, with
    original signature, was sent as directed, postmarked no later than Thursday, April 24, 2008.
    A copy of the Supervisor’s/Specialist’s letter, with all required information, is included; and the hard copy, with
    original signature, was sent as directed, postmarked no later than Thursday, June 26, 2008.
    All music teachers’ schedules are enclosed, and include clear details and explanations of anomalies.
    Criterion II:
    One copy of the “Course Offerings” form has been completed for the entire music program in the applicant school.
    Each teacher has described the form(s) of evaluation/assessment used to inform instruction, and has included samples
    as directed.
    Each teacher has prepared and labeled four (4) digital video segments, as directed, and completed the associated written
    material. Reminder: Extraneous details slow the review process, and may result in an applicant’s disqualification.
    Criterion III:
    One copy of the district’s “Scope and Sequence/Curriculum Guide” has been submitted, as directed.
    Criterion IV:
    All full- and part-time teachers are listed, and each has included “most significant” professional development activities
    and prominent professional memberships (e.g., FMEA/MENC, American String Teachers Association, American Guild
    of Organists) and/or certifications (e.g., Level I Orff, Early Childhood).
    Criterion V:
    The Elementary or Secondary Survey has been completed. Reminder: This is an “inventory,” rather than a “gatekeeper
    form.” Responding with a “No” will not jeopardize the viability of an application.

    Criterion VI:
    The repertoire list has been completed, as directed, and each entry includes a designation of either “Read” or
    “Performed.”
    Copies of programs from concerts and music productions from the current and previous years have been included.
    A digital sample of all 2007-2008 performing ensembles has been prepared, limited to two contrasting pieces per
    ensemble, and clearly labeled as directed.
    Secondary Schools: Digital or hard copies of Music Performance Assessment (MPA) sheets have been included.
    District-wide and/or All-State program covers, repertoire lists, and lists of student participants have been included, and
    students’ names have been marked, as directed.
     
    Optional: Verification of the school’s voluntary participation in the Florida Music Assessment has been provided.
    Criterion VII:
    Reciprocity of services and enrichment has been documented, and may include up to 10 clearly labeled photos.
    Criterion VII:
    A summary of a unit of study or school-wide activity that purposefully addressed literacy/reading, writing,
    mathematics, and/or science has been provided, as directed.
