SCHOOL IMMUNIZATION ANNUAL REPORTS AND SURVEYS 2007-2008
Summary:
The Department of Health requested that our office forward to all school districts the enclosed
information regarding Compulsory School Immunizations Annual Reports and Surveys for the 2007-2008
School Year. Section 1003.22(8), Florida Statutes, and Rule 64D-3.046(3), Florida Administrative Code,
require the Department of Health to collect annual reports from all public and private schools to certify
compliance with the compulsory school immunization law.
For the 2007-08 School Year, the Department of Educati
of seventh grade immunization compliance. Therefore, districts and schools may continue to use Immunization
Status Code 8 for seventh grade compliance reporting, may switch to using only Code 1 in 2007-08, or may
phase in the switch to using Code 1 as new students are entered into th
