District Health Education, Staff Training, and Education Assessment
Survey
In a continued emphasis on school district needs, health education, and staff training, Florida’s
Coordinated School Health Program (CSHP) is collecting data regarding
the status of school
districts, health education, and staff training in the state. The CSHP is supported by a
cooperative agreement with the Centers for Disease Control and Prevention (CDC) to improve
health education based on school district and school needs. Attached is a School District, Staff
Training, and Health Education Assessment Survey to help collect district data in these areas.
This survey has been provided to all CDC funded state and local projects as an example of a high
quality assessment document.
Once the evaluation has been completed, the results will be compiled and distributed to school
health education coordinators. The evaluation will highlight areas of need for school districts,
health education, and staff training and provide information that will help CSHP planners make
decisions regarding future training events and workshops. As the Health Education Coordinator
in your school district, please complete the survey and return to the CSHP by September 20,
