Annual Parent Surveys
As part of Florida’s State Performance Plan (SPP) required under Part B of the Individuals with Disabilities Education
Act (IDEA), the Office of Special Education Programs (OSEP) requires that states annually collect data from parents
of students with disabilities. Specifically, Florida is required to report the “percent of parents with a child receiving
special education services who report that schools facilitated parent involvement as a means of improving services
and results for children with disabilities.”
This year parent input will be collected via two parent survey instruments: one designed for parents of children with
disabilities in preschool programs and the other for parents of children with disabilities in kindergarten through grade
12. These surveys consist of items developed and validated by the National Center for Special Education
Accountability Monitoring (NCSEAM). Surveys will be mailed out in April 2007 to all parents of preschool children
with disabilities statewide. All parents of K-12 students with disabilities in districts with counts of students with
disabilities of less than 4,000 will receive surveys, with a sampling of parents of K-12 students with disabilities in
districts with counts that equal or exceed 4,000.
We have contracted with Piedra Data Services to conduct the sampling, mail out the parent surveys, and compile the
responses for reporting purposes. Attached is a document outlining the data fields required, file format, deadline for
submission, and contact information. A representative from Piedra Data Services will be following up with your MIS
department to ensure receipt of the necessary data. We appreciate your cooperation in assuring that these data are
released in a timely manner.

Attached is a detailed list of the specifications for the data fields. Please adhere to the enumerated
specifications, following the allotted field length and format. A reference to field definitions in the
Automated
Student Information System
(produced by the FDOE, Education Information and Accountability Services) is
included, where applicable. Each district should provide a complete data file of ONLY active students with
disabilities. Do not include students who are identified only as gifted. Please provide only one record for each
active student.
