ESE-080 Breakfast Program Supplement Report
This document is intended to assist school districts in providing high quality nutrition programs that
support the growth and development of Florida’s children. Through this and other types of assistance,
the Office of Food and Nutrition Management supports the mission of Florida’s K-20 education system
to increase the proficiency of all students within one seamless efficient system.
Attached is the General Revenue Breakfast Program Supplement Report to be used this year in reporting
the free and reduced price breakfast

that were claimed for reimbursement in elementary schools for the
period July 1, 2004, through June 30, 2005. This report should be submitted no later than August 5,
2005. Only the total free and reduced price elementary breakfasts claimed in the School Breakfast
Program can be reported. If there are elementary schools that serve students beyond grade six, only
those meals served to students in grade PK-6 may be claimed. However, NO BREAKFAST MEALS
may be claimed in middle schools.
This information will be used to determine your district’s share of the 2005-2006 State Breakfast
Supplement Appropriation. Since the distribution cannot be calculated until all reports have been
received, it is important that the report is submitted on time. These funds will be distributed in three
equal amounts, for the first three quarters. They will be included with the district school lunch
allocation.
The final quarter payment will be adjusted to ensure that the total for the year equals the number of free
and reduced price breakfasts reported, multiplied by the calculated difference between per meal average
cost and average reimbursement.
