    SUBJECT: Transition Planning for Students with Disabilities-A Guide for Families
    Educators and families are working in collaboration to assist students with disabilities to reach the
    highest levels of student achievement and successfully transition to adult life. The Florida
    Department of Education is committed to providing information to assist staff and families to meet
    the needs of students with disabilities throughout the transition process. We are pleased to enclose
    the new document,
    Transition Planning for Students with Disabilities-A Guide for Families.
    This

    Transition Planning for Students with Disabilities-A Guide for Families
    is a user-friendly document
    designed especially for families. However, others involved in transition planning, such as students
    and teachers, will also find it helpful. The purpose of the document is to provide information about
    transition planning, the transition individual educational plan (IEP), diploma options, and other
    topics. Special sections are also included to help families participate more fully in the transition
    process.
