schools to submit their mid-year reports electronically. Staff also de

and Mid-year Reports
1. Go to the Office of School Improvement website (http://www.osi.fsu.edu/)
and locate the
Quarterly and Mid-Year Report Guidance & Directions
(
PDF
format
) and click on
SIP Quarterly & Mid-Year Report Formats for F, SINI and
SINI/F Schools
(
MS Word format
).” Click on the report format for your type of
school, i.e., F School, SINI School, or SINI/F School; you will find the
report format and guidance for completing the midyear report.
2. Rename and save the
2004 2005 Quarterly Report Format
to a document
that you will complete and later attach to your on-line school improvement
plan (SIP), e.g.,

3. Using the
Guidance
document, complete the quarterly report and save it
so you will be able to attach it to your on-line SIP.
4. Once the
Sample School January Mid-year Report
is complete and saved,
go on line to your plan, the left-hand navigation bar begins with your
school name, e.g., Sample School and ends in
Quarterly Reporting
, the
new link where you will attach your quarterly report. Click on the
Quarterly
Reporting
link.
5. In the Quarterly Reporting section, you will see three sections. Once in
that section, you will see:
6. Click on the “Browse Button,” for the specific quarter, locate your quarterly
or mid-year report document for that period, select the document to
upload, click Upload and you will know the file has been attached when
the View Attached File label appears

To view the quarterly or mid-year report, click on “View Attached File.” If you
would like to make edits to the report, open the original word document on your
computer, complete your edits, save, and repeat step 6. Your original file will be
replaced by the newly attached revised document. Only one word document can
be attached to the quarterly report section; the last report attached is the
document submitted.
