4 ACTIVITY ASSIGNMENTS
5 All data reported on this form are collected to meet the mandatory requirements specified in Section 1001.03(5) and 1012.07(1),
6 F.S. These data should be reported as of November 1, 2004, or the nearest date that an auditable count can be made.

8 If a staff member has responsibility for more than one activity assignment, report that person in one activity assignment
9 according to percentage of time or importance to the district. If a staff member has equal multiple assignments, that person
10 may be reported accordingly, using decimals (e.g., .5 for Science and .5 for Physical Education).
11    
12 While most activity assignment classifications are self-explanatory, the following definitions should clarify the information
13 request.  
14    
15 Line 4 - Dropout Prevention:
16 Include dropout prevention teachers who are teaching more than one subject or a class that is competency based or self
17 contained. Teachers who are teaching mainly one subject should be listed under the appropriate subject field. Dropout
18 prevention includes alternative education, teenage parent, juvenile justice, disciplinary, and substance abuse programs.

20 Line 10 - ESOL (English for Speakers of Other Languages):
21 Do not include foreign language teachers here. They should be reported in Line 11.

23 Line 29 - "Formerly" Profoundly Handicapped:
24 The former category, Profoundly Handicapped, was replaced with two new categories: Profoundly Mentally Handicapped
25 (Line 29) and Severely Emotionally Disturbed (Line 30). Programs for autistic children should be reported on Line 31.

27 Other Classroom:
28 Include on page 2 other classroom teaching assignments not covered in Lines 1-51.

31 Include on page 2 non-classroom instructional assignments not covered in Lines 52-66.

33 GRADE ASSIGNMENTS

42 Include in these columns newly hired personnel who are certified or certifiable according to the requirements specified in the
43 state Course Code Directory for the field for which they are hired (including beginning teachers who are certifiable). For the
44 purpose of this survey, certifiable refers to subject field qualifications. Do not designate as not certifiable teachers who are
45 considered out of field because they lack ESOL (unless they are teaching a class in ESOL) or middle school endorsement.

48 Include in these columns newly hired personnel not certifiable in the appropriate area, including newly hired, long-term
