Please use the questions listed below to compile your responses and gather necessary
information before you log on to complete the survey.
Has your local board of education adopted a formal written policy concerning
HIV/AIDS prevention education?
Are there any current plans to develop a new policy or revise an old policy in this
area?
In which grade is HIV/AIDS education required or recommended for students in
one or more schools in your district?
What percentage of all students in elementary school, middle school, or high
school would you estimate receive HIV/AIDS education each year?
How would you describe your district’s HIV/AIDS curricula at the elementary,
middle, and high school level?
