SCHOOL DISTRICT LETTERHEAD
(Date)
Dear Parent or Guardian:
This letter serves to inform you that your child
may
be eligible to participate in the
John M. McKay
Scholarship for Students with Disabilities Program
, commonly known as the McKay Scholarship
Program. This program was created to provide enhanced educational options to parents of students with
disabilities. Currently, more than 12,000 Florida students are participating in this program.
In order for your child to be eligible for the McKay Scholarship Program, he/she must meet certain
requirements. Your child must have been enrolled in a Florida public school for the 2003-2004 school year
and reported by the school district for funding during the preceding October and February student
membership. Additionally, your child must have an active IEP (Individualized Education Plan) from the
public school.
If your child meets these requirements, he/she may be eligible for the program and you may:
•
Request a different public school within your district;
•
Request a public school in an adjacent school district;
or
•
Receive a scholarship for your child to attend a participating private school.
To learn more about your child’s educational options, please contact your school district’s Parental Choice
Office at _____________________________. You may also contact the Department of Education, Office
of Independent Education and Parental Choice Information Hotline at 1.800.447.1636 or visit the website at
www.opportunityschools.org
and select the
John M. McKay Scholarship Program for Students with
Disabilities Program
option.
In order for a student to be eligible for all four payment periods, intent to participate in the McKay
Scholarships Program must be declared by July 1, 2004.
Again, this letter serves to notify you that your child
may
be eligible to participate in the McKay
Scholarship Program.
This letter does not guarantee your student’s eligibility.
Sincerely,
