    SUBJECT:
Use of FCAT Preidentified Student Labels
The purpose of this memorandum is to remind districts about the use of pre-gridded labels for
FCAT. Preidentified student labels MUST be used on student answer documents when such
labels have been provided. The Department will be monitoring the use of these labels on
materials returned for scoring. Schools that have a high percentage of student documents
without labels may not receive a school grade until the district is able to review and improve the
accuracy of the information garnered from the hand-gridded answer documents.
Please refer to pages 10-12 of the
2004 FCAT Reading, Mathematics, and Science Test
    Administration Manual
for specific directions regarding Preidentification of Student Answer
Documents. (A copy of the 2004 test administration manual may be downloaded from the
following site: http://ncsschoolhouse.com/fcat.) As indicated in the manual, if an individual
label contains inaccurate student data, it should not be used.
If you have any questions regarding the use of student labels, please contact Victoria Ash or
LeeAnn Brown at 850-245-0513 or SunCom 205-0513.
    Questions relating to school grades
should be directed to Sue Klos at 850-245-0411 or SunCom 205-0411.
