    In our continuing effort to improve our service to you, we need your feedback. At the web site
    link below, you will find a brief but very important survey instrument. Simply, click on the link
    and follow the instructions at the survey web site. When you are done with the survey, click on
    the Save button at the bottom of the survey. Your survey responses will be automatically saved
    to a database from which we can retrieve the data you input on the e-survey.
    The data we collect from this survey is important because it will give us first-hand, unfiltered
    information about the kind and degree of problems, issues, and barriers that you, our customer,
    have had to endure this past year. This information can then be used to develop strategies and
    techniques to improve our funding processes this coming year. It would also be helpful if you
    would provide the name of your program manager, or other DOE point of contact you have dealt
    with, this past funding year so we can identify our strengths and weaknesses as a staff.

    Please take a few minutes to complete this survey, and where appropriate, give us specific
    examples to support your ratings, as well as, any ideas or suggestions you have on how we could
    provide better service to you. Specifically, we’re looking for ways to improve the Workforce
    Education Funding Teleconference and Drive-In Workshops, Request for Proposals, project
    reviews/evaluations, performance reporting and invoice forms, budget amendment processes,
    and technical assistance needs you might have.
