Please read the following instructions before completing the registration form. Questions concerning the conference

• All mailed pre-registrations must be postmarked by September 23, 2003 for receipt by September 26, 2003. ($10 reduced fee by August 15, 2003).
• Make checks payable to the Florida Association for the Gifted.
• Mail the registration form and check or purchase order for the conference

• The number of participants is limited. All registrations will be handled on a first-come, first-serve basis.

Do not mail any registrations after September 23, 2003, but bring the form to the conference for on-site processing.

On-site registrations carry a $10.00 surcharge and will be limited by available seating.

• Registration begins at 7:30 am Friday morning.
• Registration fee includes Friday lunch, Friday afternoon Wine & Cheese Auction & Social, and Saturday brunch.

"Members" are those with current membership in FLAG as well as new or renewing members who pay dues now.

Your membership expiration date is on the mailing label of this flyer and other correspondence from FLAG.

• Non-members are those who are not members and do not wish to become members.
Refund requests must be made in writing to the address given above and will be honored until October 6, 2003.

After October 6, 2003, refund requests will be subject to a $25.00 cancellation fee.

Refunds will be issued after November 21, 2003.
