    Attached are the General Assurances for Participation in Federal and State Programs of the Florida Department of
    Education. The General Assurances currently on file with the Department were developed in 1998, and have been
    revised with the issuance of the 2003 “Green Book “ which becomes effective July 1, 2003.
    It is required that you certify compliance with these General Assurances prior to the issuance of any projects from the
    Department.
    This year your certification of compliance with the General Assurances for Participation in Federal and
    State Programs of the Florida Department of Education will be in effect for the duration of your participation in Federal
    and State Programs or until such time as the requirements change.
    Please print, sign and date the certification and return both the certification and attached assurances by July 15, 2003, to
    the address below. Please keep a copy for yourself.
